- Representation and Description - I hereby represent that I am the custodian for, or authorized to act on behalf of the custodian for, the item(s) being worked on.
- Responsibility and Request for Service – The computer equipment I bring in requires services to affect a repair, or to modify its hardware/software configuration. Recommendations for service made by The PC Doctor’s staff may be provided to me as guidance for what repairs or modifications should be made. Full responsibility for what repairs or modifications are to be made rests solely with me. I hereby request that The PC Doctor’s staff act on my behalf to perform the services referred to in this paragraph.
- Billing Rates for Service – Payment is required in full at the time of pickup/receiving of items listed above. Finance charges will accrue for non payment at the monthly rate of 15% of the total bill if payment is not made in full. Any items left at the premises of The PC Doctor for more than 30 days after completion of work will be considered property of The PC Doctor. Checks returned for non payment will be subject to a $25.00 service fee.
- Warranty for Service – Any warranty as to the likely successful outcome of hardware/software services provided to me is denied. The PC Doctor or anyone associated with will not be held liable for any damage, data loss, or loss of sales, profits, income, or legal issues that may occur due to data loss to this equipment that may have existed prior to, or was the result of any service action by The PC Doctor or anyone or company associated with. All work is guaranteed 7 (seven) days after pickup/receiving of computer equipment. No guarantee is provided if hardware/software configurations or setting changes are made after pickup/receiving of computer equipment. No warranty is provided for either new or used computer hardware components or computer systems.
